Before beginning services at Guzman & Baker, you will be asked to review and sign a consent to treatment and related policy documents. This page summarizes the key terms that govern your care. Your intake paperwork will contain the full version of each policy. By scheduling and attending services at Guzman & Baker, you consent to receive mental health evaluation and treatment from our licensed clinicians. This includes individual therapy, family therapy, or child therapy, as agreed upon between you and your provider. You have the right to:
  • Participate in planning your treatment
  • Ask questions about your diagnosis, treatment approach, and progress
  • Decline or discontinue any treatment at any time
  • Receive care that is free from discrimination
Treatment at Guzman & Baker is voluntary. You may end services at any time. If you choose to discontinue, we encourage you to discuss this with your therapist so we can support a thoughtful transition.
For minors receiving therapy, a parent or legal guardian must provide written consent for treatment. The specific rules around what information parents may access in a minor’s therapy records vary by state and will be explained during intake.

Cancellation policy

We ask that you provide at least 24 hours’ notice if you need to cancel or reschedule an appointment. This allows us to offer your appointment time to other patients who need care.
  • Cancellations made with 24 or more hours’ notice: No charge.
  • Cancellations made with less than 24 hours’ notice: A late cancellation fee may apply.
  • No-shows (missed appointments with no notice): A no-show fee may apply.
Late cancellation and no-show fees are generally not covered by insurance. These charges are billed directly to you. The specific fee amounts are listed in your intake paperwork.
Exceptions may be made for genuine emergencies at the discretion of your provider. If an emergency prevents you from giving adequate notice, contact your provider as soon as possible.

Payment and fees

Payment is due at the time of service. We accept most major credit and debit cards. We also accept Health Savings Account (HSA) and Flexible Spending Account (FSA) cards. Insurance billing: We accept Blue Cross Blue Shield, Aetna, Cigna, and United Healthcare. Your out-of-pocket cost (copay, coinsurance, or deductible contribution) is due at each appointment. It is your responsibility to understand your insurance benefits. We recommend verifying your coverage before your first session. Self-pay: If you do not have insurance or your plan is not accepted, self-pay rates are available. Contact our office for current rates. Outstanding balances: Accounts with outstanding balances may require payment before additional appointments are scheduled. If you are experiencing financial hardship, please speak with our administrative team. We will work with you to find a solution where possible.

Telehealth terms

Guzman & Baker offers telehealth sessions via a HIPAA-compliant video platform. By participating in telehealth, you agree to the following:
  • You will be located in the state where your provider is licensed during all telehealth sessions.
  • You will participate from a private location where you can speak freely and confidentially.
  • You understand that technology limitations (such as connection issues) may occasionally disrupt sessions.
  • You will not record sessions without the written consent of your provider.
  • You will have an emergency plan in place, including a local emergency contact and the address of your location, which you will share with your provider at the start of each session.
If your telehealth session is interrupted due to a technical issue, your provider will attempt to reconnect via video. If reconnection is not possible, they may continue the session by phone or reschedule, depending on the circumstances.

Good Faith Estimate (No Surprises Act)

Under the No Surprises Act, you have the right to receive a Good Faith Estimate of the expected costs of your care before services begin. This applies if you are uninsured or if you choose to pay for services without using your insurance. A Good Faith Estimate:
  • Is provided before your first appointment upon request
  • Lists the expected cost of the services you will receive
  • Is not a final bill — actual costs may vary
If you receive a bill that is $400 or more above your Good Faith Estimate, you may be eligible to dispute the charge. For questions about your Good Faith Estimate or to request one, contact our billing team.
Even if you have insurance, you can request a Good Faith Estimate for sessions where you plan to self-pay or apply charges to your deductible. This helps you plan your healthcare budget in advance.

Updates to these terms

Guzman & Baker may update these terms periodically. If there are material changes to policies that affect your care, we will notify you in writing. Continued use of our services after notice of changes constitutes acceptance of the updated terms.
