We ask that you provide at least 24 hours’ notice if you need to cancel or reschedule your appointment. This allows us to offer your time slot to another patient. Cancellations made with less than 24 hours’ notice may be subject to a late cancellation fee. The fee amount will be equal to 100% of the total cost of the scheduled appointment. Your insurance plan typically does not cover late cancellation fees, so this charge would be billed directly to you. If you don’t contact us, or you don’t show up within 15 minutes of the scheduled time, we will need to charge your credit card on file the full amount for the reserved time. If you need to cancel, please contact your provider as soon as possible rather than waiting until the last minute. However, if an unforeseen situation arises—such as illness, bereavement, an accident, etc.—we will waive the fee if you can provide documentation that supports it. Health insurance doesn’t cover missed appointments, and Guzman & Baker depends on timely, full payment to provide the highest quality of care to all of our patients.
