Guzman & Baker accepts UMR insurance plans. Your UMR mental health benefits may cover individual therapy, family therapy, and child therapy sessions at our practice. This page walks you through how your coverage works and how to confirm your benefits before your first appointment.

How UMR coverage works here

Guzman & Baker submits claims directly to UMR on your behalf. You pay your cost-share at the time of service. The amount you owe depends on your specific UMR plan and where you are in your plan year. Your costs may include:
  • Copay — A fixed per-visit amount you pay for each outpatient mental health session.
  • Coinsurance — A percentage of the allowed cost you pay after your deductible is met.
  • Deductible — The annual out-of-pocket amount you must reach before UHC shares in the costs.

Common UMR plan types

UMR (PPO)

A flexible plan that lets you see any licensed provider. You pay less when you see in-network providers like Guzman & Baker, and more for out-of-network care.

UMR (HMO)

Covers care within a designated network. You may need a referral from your primary care provider. Confirm with UMR whether a referral is required before your first session.

UMR (EPO)

Covers services only from providers within the UMR EPO network. Out-of-network care is not covered except in emergencies. Verify that our practice is in your specific Navigate network.

UMR HDHP / HSA-eligible

High-deductible plans often paired with a Health Savings Account. You typically pay the full allowed cost per session until your deductible is satisfied.

Using your UMR member ID

Your UMR member ID appears on the front of your UMR insurance card. This number identifies your plan and links your claims to your account. You will need it to:
  • Call member services to verify your benefits
  • Access your account on umr.com or the UMR Health app
  • Provide to our office when scheduling or submitting paperwork
If you do not have your physical card, you can access a digital version of your UMR ID card by logging into umr.com or downloading the UMR Health app.

How to verify your UMR benefits

Before your first appointment, take a few minutes to confirm your mental health benefits. This prevents billing surprises.

Find your UMR member ID card

Locate your UMR insurance card. You will need your member ID, group number, and the member services phone number printed on the back.

Call UMR member services

Call the behavioral health or member services number on the back of your card. Ask the representative about your outpatient mental health benefits.

Ask the right questions

When you reach a representative, ask:
  • Is Guzman & Baker Behavioral Health Services (NPI: 1669265328) in my network?
  • What is my copay or coinsurance for outpatient mental health visits?
  • Do I have a deductible, and how much have I met so far?
  • Do I need a referral or prior authorization for therapy?
  • Are there any limits on the number of sessions covered per year?

Check umr.com (optional)

Log in to your UMR member account online to review your summary of benefits, track your deductible, and search for in-network providers.

Share your findings with our office

When you schedule, let us know what you learned. Our billing team can also help verify your benefits before your first session or answer additional questions.
UMR manages behavioral health benefits through Optum in many plans. When you call, you may be transferred to Optum for questions about mental health coverage. This is normal — Optum administers the behavioral health portion of many UMR plans.

What to complete prior to your first appointment

Submit the following in your secure patient portal prior to your first session:
  • Your UMR insurance card (front and back)
  • Complete additional intake forms (sent to you before the appointment in your patient portal)
  • A form of payment for your copay or cost-share amount
Some UMR plans require prior authorization before therapy begins. If your plan has this requirement, contact our office as soon as possible. We can work with you to submit the request so authorization is in place before your first session.
